bad swivels or bad line ? or rookie mistakes?

When you spooled the reel did the line come straight off the spool or off one of the ends of the line spool? If the line came off one of the ends that would be a reason s for that...

Watch out for jelly fish. Jelly fish goo can jam up the best swivels. Once the line has a twist in it, it can be done. Happened to me last year.
